Team,
I have the same issue...

Flyinside Vive v1.9 [Just purchased the P3D v4 version]
P3D v4.1.7.22841 (Academic)
VRS TACPak v1.5.0.1

Also, I get a SODE error when P3D loads, It seems that Flyinside bumps SimObjects SODE outside of P3D in task manager so that it doesn't close correctly and needs to be killed manually... PFA screenshots

Without Flyinside loading

Appreciate the help. :)
Thank you very Much,

I am having the exact same problem. Just with my Oculus ..

Background
--------------
Which version of Fly Inside P3D do you use? -- FLYINSIDE_P3D4_PRO_1.9_OVR_1300.exe
Which version of P3D do you use? -- 4.1.7.22841
What P3D plugins do you use? -- All removed
Do you use reshaders ? -- No
Please upload the log file. -- As attached
Sometimes it also makes sense to upload the log file from P3D. -- Na
Are your drivers up to date? -- Yes Nvidia 1080Ti
Are you running our software on a laptop? -- No , on a Desktop
Is your HMD plugged directly into the HDMI-port of the accelerated GFX-card? -- Oculus Rift plugged into HDMI on the 1080Ti

Specs
------
Processer - AMD FX(tm)-4300 Quad -Core Processor 3.80 GHz
Memory - 8.00 GB
System - 64 bit Windows 10
P3D - FlyInside P3D4 Pro 1.9 (trial version)
VR - Oculus Rift CV1

Symptoms
------------
- P3D starts correctly standalone no problem. Even the built in VR works fine (albeit with a very low frame rate)
- I install FI 1.9 trial version and follow installation prompts
- There are a few prompts to restart FI, which I do
- P3D stops with "Prepar3D exe has stopped working - A problem caused the program to stop working correctly. Windows will close the program and notify you if a solution is avaliable"
- Attempts to even start P3D standalone results in the same error.
- I have to uninstall FI 1.9, and then P3D will start again.

Note: I see that I have the same last 2 lines in the logfile as captflyby:
8.63023 VRSLI\SLIManager.cpp 58 Creating multi-GPU mask
8.63041 VRSLI\SLIManager.cpp 92 Failed to create MultiGPUDevice

..and we have the same Video Card 1080ti.. not sure if it's related.

FYI - I have FI also installed for FSX , and this works fine.
I thought that it may have had something to do with my Oculus runtime, but captflyby is using an HTC Vive and has the same problem.

FYI - if I reset the settings , and instead of selecting my 1080Ti as the graphics adapter for my Headset - I select "Microsoft Basic Render Driver", then it gets me past the point of crashing and allows P3D to get to the initial scenario screen.
Of course, the next message is "The Oculus Rift could not be found on the selected adapter....", but I thought that may be a clue to what is going on.
I double checked the driver on my card, and it's up to date..

Any ideas? I'm keen to get FI up and running as native VR on p3d v4 is basically unusable.

OK guys.. problem sorted.. I have 3 monitors and the Oculus. 2 Monitors and the Oculus were plugged into the 1080ti. For some reason when I traced the 3rd monitor, it was actually plugged into the Motherboard! My bad.. plugged all monitors and Oculus into the 1080ti, and problem solved.
